Yes, the Medication Consent Form must be completed whether they take medication or not.
Yes, the school district may have paperwork that could resemble our paperwork, but it is important to note that their paperwork is not shared with the school.
Paperwork completed in our Early Intervention program in White Plains may mirror our paperwork here in Yonkers, but we must have new forms completed prior to the start of the Preschool program.
No, if you have completed the prescription for the district, we ask that you kindly forward a copy of the completed prescription to info@fredskeller.com. Failure to do so may result in delays to the start of your child’s services.
If you’re child is in the Early Intervention program in White Plains and it is not expired then yes please let us know in an email at info@fredskeller.com. If it is expired, please send us an email with the appointment card.
Yes, if one or the other does not apply you must fill out the form entirely and write “N/A” on the form.
